Elle s'est fait connaître grâce au rôle de l'infirmière Mildred Ratched dans Vol au-dessus d'un nid de coucou (1975) de Miloš Forman, pour lequel elle remporte l'Oscar de la meilleure actrice, le Golden Globe de la meilleure actrice dans un film dramatique et le BAFTA de la meilleure actrice.
